Provides case management services to injured persons to help them manage life as they heal.
Northern Brain Injury Association case management services combine assessment, planning, facilitation, care coordination, evaluation, and support for options and services to best meet client and family needs through communication and assistance to access available resources in order to promote maximum independence, and optimal recovery. Case managers assist with:
